Request form for Bowland Maths DVDs - for use in England only
 



The DVD ROM contains the Bowland Maths materials: 18 Case Studies and 5 Professional Development Modules, together with a supporting booklet.

Three to five DVDs can be supplied for each school with KS3 maths classes - with the number depending on the numbers of KS3 maths classes. For Local Authority schools, requests for the DVDs should normally be made by an appropriate person within the Local Authority - for example, the Secondary Maths Advisor/Consultant. Other schools in England should complete this form as appropriate.

The DVDs will be despatched within one or two days of the request being received.

'Education Direct' will distribute the DVDs; they will hold your data for the purposes of the administration and distribution of the Bowland Maths DVDs only. Your data will not be passed to third parties (other than the Bowland Trust) without your consent. Your data will be destroyed once it has been inactive for a year.
